Other uses and disclosures not covered by this Notice or applicable laws will only be made with your written authorization. If you provide an authorization, you may revoke it at any time by submitting a written revocation to our Privacy Officer. Disclosures made in reliance on your authorization before revocation will not be affected.

Your Rights Regarding Your PHI

You have the following rights, subject to certain limitations:

  • Inspect and Copy: You have the right to inspect, receive, and copy PHI that may be used to make decisions about your care or payment. We have up to 30 days to make your PHI available and may charge a reasonable fee for copying, mailing, or other supplies. We may deny your request in certain limited circumstances, in which case you have the right to have the denial reviewed by a licensed healthcare professional.
  • Electronic Copy: If your PHI is maintained electronically, you have the right to request an electronic copy of your record be given to you or transmitted to another individual or entity.
  • Summary or Explanation: We can provide a summary of your PHI rather than the entire record, or an explanation of the PHI provided to you, so long as you agree to this alternative form and pay associated fees.
  • Request Amendments: If you believe PHI we hold is incorrect or incomplete, you may request an amendment for as long as the information is kept by or for us.
  • Accounting of Disclosures: You have the right to ask for a list of disclosures we have made of your PHI. The first accounting within any 12-month period will be free. Submit your request in writing to our Privacy Officer.
  • Request Restrictions: You have the right to request a restriction or limitation on the PHI we use or disclose for treatment, payment, or health care operations. We are not required to agree, but if we do, we will comply unless the information is needed for emergency treatment.
  • Request Confidential Communications: You may request that we communicate with you only in certain ways, for example by mail at a specific address or by phone at a specific number. Submit such requests in writing.
  • Receive Notice of a Breach: You have the right to be notified upon a breach of any of your unsecured PHI.
  • Receive a Paper Copy: You have the right to obtain a paper copy of this notice upon request.
  • File a Complaint: If you believe your privacy rights have been violated, you may file a complaint with Monaco Health or with the Secretary of the U.S. Department of Health and Human Services. You will not be penalized for filing a complaint.

Changes to This Notice

We reserve the right to change this Notice. We reserve the right to make the changed Notice effective for PHI we already have as well as for any PHI we create or receive in the future. A copy of our current Notice is posted in our office and on our website.

Compounded Medication Disclaimer

  • Compounded medications are tailored to meet the specific medical needs of individual patients when commercially available, FDA-approved drugs are not suitable.

  • Compounded medication is prepared by a licensed 503(a) or 503(b) pharmacy in accordance with applicable federal and state laws. Compounded drugs are not approved by the U.S. Food and Drug Administration (FDA).

  • Please be aware that compounded drugs may pose higher risks compared to FDA-approved products, including potential variability in strength, quality, and purity.
